Default IceVision v2
This theme is called IceVision v2.

Now this skin runns on TS theme system you need to install it.

You can find it here - here & here: http://www.bvlist.com/mods/6925-themes-system.html

To make your tracker look exactly as mine does in the screenshoot you will have to edit your index.php - browse.php - upload.php and forums.php. These files will be included in the zip file.
